This female patient underwent facelift and browlift cosmetic facial rejuvenation, after which she developed a serious scalp infection in the region of her browlift incision line. The patient was managed by her cosmetic surgeon over a prolonged period during which antibiotics and local wound care were used to clear the infection. She was, however, left with an unsightly large scar in her frontal hairline region. Her examination upon referral to my office revealed a very dense, full-thickness, frontal scalp scar. The scar tissue lacked mobility and was fixed to the underlying bone. The patient was quite distraught over the disfiguration that resulted from her cosmetic facial procedure. She was also extremely concerned that additional surgery could worsen the situation. Various options for repair were discussed with the patient, and it was decided that a staged grafting approach would best suit her personal needs and concerns.
A staged repair using two sessions of relatively low-density grafting was recommended because of concerns relating to poor circulation within the large scar zone. The graft zone received a total of 1500 grafts in two sessions that were separated by one year. Presented here are 10 month postop photos. It is interesting to note that the overall color, texture, and mobility of her original scar were significantly improved by the addition of healthy soft tissue into the troubled zone.
